This question surprised me too, but I am positive, I did not have this question the first time, I started Kana.
When I got it the first time (that was the second time, I ever started Kana), I chose "I had a sister" and I had to face my nemesis again (ending#5), so I immediately quit back to the main menu and did, what I wanted to do in the first place: Play the game again from the start by selecting "I have a sister" (and got to ending#2). Well, two weeks have passed since then and I was going toplay Kana tonight again anyway.

Maybe, the hidden ending (if it exists) hides behind "I had a sister", after all other endings have been seen?
Or this selection starts just one random ending out of the already experienced ones?
